Application for New Water Service Connection
Service can be availed daily from 8:00 AM – 12:00 Noon and 1:00 PM – 4:00 PM
Step 1
- Applicant / Client: Fills up Application Form
- Activity: Assist the Client in filling up of the Application Form
- Requirements: Application Form
- Accountable Person: Customer Service Assistant / Administrative Service Aide
- Duration: 10 minutes
Step 2
- Applicant / Client: Waits and receives the schedule of Orientation for New Applicants
- Activity: Check the application form and give schedule of the Orientation for New Applicants
- Accountable Person: Customer Service Assistant / Administrative Service Aide
- Duration: 5 minutes
Step 3
- Applicant / Client: Pay to the cashier the required registration and service fees
- Activity: Give to the cashier the accomplished application forms / Issues Official Receipt
- Requirements: Application Form / Official Receipts
- Accountable Person: Customer Service Assistant / Administrative Service Aide / Cashier
- Duration: 10 minutes
